For patients with chronic conditions that need follow-up only 1-2 times per year, fee-for-service may be appropriate. Which model of care is most appropriate for you will be discussed at the initial visit.

For patients in the fee-for-sevice model, any clinical question or lab result will require a visit with Dr. Dinh.

This charge will include:

• Review of labs and images obtained for the visit (recommend to get labs done before the visit when possible)

• 30 min clinic visit, in person or telemedicine

This charge does NOT include the following:

• Prior authorization requests (25$ fee)

• Access to the patient portal

• Additional phone conversation with Dr. Dinh (charged at 300$/hour, though most conversations are under 10 min)

• CGM/Pump download (50$)

• CGM placement with training (100$)

• Any additional forms/letters needed (25$ each)
